Annoying user able to register with a blank username and make blank threads

Hi all,

I've had an annoying user that we had to ban go on a rampage today. Unfortunately he is pretty smart with computers and has found some sort of way to exploit the registration criteria as well as the general posting criteria. At the bottom of my main forum page, it said "Welcome to our newest user, "

Anyone seen this before or know what to do about it?
