1. It looks like he's just modified Cyb's subforum manager (it's called something like that) to put the subforums in their own box.

2. Just add the image to your template. Give them names like fx.png where x is the forumid. So, for forum 2, it would be f2.png and then just enter the image source as <img src="f$forumid.png" etc... /> in the template. Or, there may be a mod to do it - try search.

3. Probably just an edit to the template but I don't know the specifics. You can probably figure it out by looking at the page source.
