I am looking for some sort of plug in that will enable one of my forums to be used as a feedback mechanism for our companies training courses.

I would like to be able to send the students to a particular forum and have them fill in a pre defined form rating their training experience. I would also like them to be able to generally enter comments as well.

The idea is for the content to be visible to all, so they can see how good our courses are.

Does anybody know if such a plugin exists?
