They appear on the "currently active viewers" list at the bottom of that forum even though they are not accessing that forum, do not have permission to access that forum and the access_log shows that they've never tried to access that forum.

If I disable this mod then this stops happening. If I change the last forum on the list to a different one then the phantom viewers go to that forum. It's quite possibly the side effect of a different mod but switching this one on and off causes it to manifest itself.

Any pointers welcome!

i have this same problem, im used vb 3.6.8 patch lvl2, i have private subforum only for mods,admin, when this hack is enable i see in our subforum users who search something on forum, if i disable hack problem gone.
