I want the title to link to the converted code instead of linking to original site

example
Take megavideo url : hxxp://www.megavideo.com/?v=39HCOWKJ

The present title links to same url: hxxp://www.megavideo.com/?v=39HCOWKJ

but i want it to link it to hxxp://www.megavideo.com/v/39HCOWKJ.30430295.0

How do i, do that

AND more over when i use two youtube links in single post the embed player is getting displayed 2 times each
