For the first problem (username being once used and changed) your problem is not 'with this mod' but with your vBulletin settings actually. Located in the vBOptions under
User Registrations Options you have a setting called
Username Reuse Delay that is set to 365 by default meaning no previously used username can be used within a year after the last day the last one was used. Setting this to 0 should solve your problem.
The code uses
default vBulletin functions to check usernames. The code does not bias based on if someone has requested it already, and this task will get even more complex in the next version.
