A few items to examine -

In your admincp, under Downloads - Create Threads -> Settings, is the Default UserID set as either "-1" or to a valid userid?
Does the user trying to add the file have permissions to create the thread in the designated forum?
If the category is set to go to a specific forum, is the category using a valid userid?
