Sounds like you've uploaded the images directory incorrectly or not at all. Make sure that it's in the proper location (the root directory of your forum), and that should fix your problem.

I'm not really sure that I understand what you're talking about here. Is this a mod you're adding? The hooks for the vBulletin ad locations should all be there in the templates already.
