1. I doubt he has a static IP, 90% of end users have dynamic IPs now. So telling him to only use the IP he registered with would be pointless.

If you can detect it you can ban the IP. And you can only detect it if the proxies goes the proper route and identifies itself as a proxie which most do NOT.
3. I would never suggest blocking proxies, you could potentially lose traffic that way. Every work/school network in the world is trying to block any site its users want to use these days, if your site gets flagged as one of those sites the only way people will be able to reach your site from school/work is through proxies.
