Notice the output is not outputting proper numbers

You need to construct the sentences your self. Example:

<phrase 1="$post[nominate_topic_nominated_times_formatted]" 2="$post[nominate_topic_nominated_posts_formatted]">$vbphrase[nominate_topic_times_posts]</phrase>

will construct a phrase: Nominated x times in y posts.
with x = value of $post[nominate_topic_nominated_times_formatted]
and y = value of $post[nominate_topic_nominated_posts_formatted]
