1.
If you want to have the Mod active only on one forum, there is an option for it:
"Unique Forum On Which This Mod Must Be Enabled"
On all remaining forums, you will not see the nomination button.
2.
If you want to select more than one forum on which the Mod must be enabled, i.e. you want to have it disabled on other forums, there is an option:
Individual Forums On Which This Mod Must Be Disabled
Do not mix the two options!!! If a unique forum option is specified, the second option will be over-ruled.

The postparsed table of vBulletin is updated when a new post is made, when a post is edited/updated, as well as when a post is viewed, if a record doesn't exist for that post in the table.

It is automatic. It will display the award winners and posts awarded at the end of the contest.
In the ACP, you can disable displaying it's link in the Quick-Links navigation
