I use Dream's Chatbox....low server loading, and it has an Ajax autorefresh where you can specify the time between refreshes, or switch the refresh off altogether.

I've been running it HERE for ages now with absolutely no performance issues whatsoever.

As well as being good for server load, it's also the slickest and most professional of all the shoutbox addons. It's been tweaked frequently by the author until it's near-perfect.
