This is an inbult feature of vBulletin 3.5, where you can add user notes by going to a user's member profile.

To be able to add notes, go to AdminCP -> Usergroup Manager and edit the usergroup you wish to be able to post notes about users. Halfway down the permissions should be User Note Permissions which you can customise to your own desire.

May not be exactly what you are looking for, but throwing that out in case you weren't aware.
