For that, all you would have to do is put the "<!-- insert affiliate box -->" above the "Mark as read" section of the code. But it will probably look funny. You can move it above your users if you'd like, you would just have to move the "<!-- insert affiliate box -->" right below "<!-- what's going on -->".

And no, there's no size restriction. I personally like having multiple small buttons (88x31 is normal affiliate button size) but you can have whatever size fits your site best.
