Oh, okay I missed that. I personally use this modification on my own board which is running 3.6.5 - and is working just fine with no ill effects.
The majority of modifications released for vBulletin now-a-days are primarily product based, which means that you upload a file and it does all the work for you -- leaving little room for error on your part; this also means that it can just as easily undo its work. If you look in your AdminCP near the bottom you'll see "Plugins & Products" and under that "Product Manager"; every time you install a modification that uses a Product XML file, it will be displayed here and can be Enabled, Disabled, or uninstalled.
While not necessarily required, setting up a test server and performing modifications on it before your live board is often a good practice -- for obvious reasons. But even if you make a mistake in installing something, it's most often quite easy to fix.
I have both a local and server-based testboard for my forum; I do this mainly because I don't always have access to an internet connection. The choice is yours which route you decide to go. Setting up a testboard on your server is easier than doing it locally primarily because your server already has the necessary software installed to support a website: PHP, mySQL, Apache, etc.
By the way, I would give the Advanced Sticky Options modification a try on your forum, it might just be that one user who is having trouble with it on his board - you never know. If it doesn't work, it can easily be uninstalled.
