Did this happen after an upgrade? I've done upgrades before and had pages stop working and found out that the first or last line of the page had some extra unseen characters added in the downloaded pages. So, I copied the line from a page that worked and then it was fine. It was over a year ago, so I don't remember the specifics, sorry.

Oh, how about the permissions on your attachments page (and scripts it calls). Attachment.php should be "755"
