If you can't get it working, it means that the first if(...) instruction from plugin is false.
Go to Plugin edit and go through these conditions. There is definetely something, which is being hacked or changed compared to the original.
I have also changed this code a little to reflect my changes and it works.
BTW: adding
&postid=$thread[postid] to link to the attachment would be an option for some of you, I think
