Hello all,

I was wondering if anyone could offer some tips as to how I could install something similar to what's been done on this site in the the Mods threads.

Basically, I'd like to have something similar to the Mods threads in this forum (see image below) in one of my forums just with different fields and stuff. The key things I'd like to have are:

1. input fields that users have to fill out when they submit a hack
2. an install button that tracks how many people have used a hack

If anyone could offer any guidance I'd be most appreciative.
