I just went through all the steps to upgrade from 3.7.4 to 4.0 and the very last step tried to go to my admincp and said this:

I believe it will "alarm" only once per user that has more than one account. If you cleared everything, it's probably detecting all of the users it already detected before the fresh install.

Are all your forum files on the root directory or do you have them in a sub directory on your server? I'm not an expert but I think he's saying that he has his forum files in a folder titled "forum"...
