I just tested it and your plugin works as intended. The problem is when a thread is submitted by a guest using easy forms.
I am not sure if you are familiar with the easy forms...

We seemed to have lost the image verification field for guests. It produces an error when submitting saying the image did not match but it is nowhere to be found.
