Its been broken since day 1, developer isn't going to be fixing it. Someone else needs to delve into the code either to rewrite how its done so it shows up properly or just ignore it.

Turn off username checking. Since bots use fairly common names sometimes, real people will get blocked. Just keep IP and email address checking on, turn on human verification in your VB settings...

I wouldn't test on a live forum, could always backup and restore to a new DB and build your forum up...
