SQL logs

I am not sure if something like this exists, but I figure this is a good place to start.

There have been some problems with our server and things getting changed in mySQL, either via phpmyadmin or command line.

Is there a loggind tool that can keep track of sql commands, what they were and give the IP information?

Any suggestions would be great.
