How can I fix the register.php code?

When someone signs up, they see this after hitting submit:

and I get this email:

I believe this happens when you don't properly upgrade vB versions. Did you run the upgrade script in addition to modifying files?

(side note: although I know you didn't write the code, it's bad practice to not specify column names for this very reason, among others.)

I ran a hack that didn't work (can't remember which one)....is there any way to take this out of my database?

Assuming you upgraded to 2.3.0, you failed to run upgrade23.php. Run it and it will add this field.
