Members names being mixed up
 
As far as I know this has only happened twice but that is just from the people who has said something to me.

First about a month ago one of the mods made a post and the post came out with another users name and info (avatar,name,sig ect)

Now someone just logged in and told me that the board said welcome so and so but it was another members ID and not his.

Has anyone seen this before?

It can happen behind proxies. There really isn't much you can do about it. Make sure every user has cookies turned on in their user CP and their browser, and in the case of IE6, that their privacy protection setting doesn't exceed Medium.
