I've been searching, but must not be putting in the proper key words .. where would I find out how to make the attached box.

Since I've found 100s of threads asking where this is .. I thought I would post it (it took me at least 5 hours to find the information) .. so perhaps I can save someone else a little time:

The two hacks I've used are:

I'm still working on the formating .. but I've got it working (although I'm not sure how this will look for guests and those registered but without an avatar .. I'll need to check that on another computer).
