PM on Infraction

Since upgrading to 4.2.2 (PL4) I had to issue my first infraction. However, I was not able to add a PM to the user to explain the action, even though the setting "Require Infraction Message" is set to Yes. Also, a copy of the automatic notice did not go to my PM box as it usually does. Any suggestions?
