Mods for Report Card

Hi all,

I'm trying to create a report card system on my forum. Can you think of any mods that are easy to convert into one?

Thanks for asking my friend. Something like this:

Code:

Student Report

Username      Class of (profile field)      Quarter: (text field)



Subject: (Drop Down Menu)



Class/School Standards(Drop down menu): (5 star selection)


(Drop down menu): (5 star selection)


Comments: (textbox field)



Send as PM to: (text field, separate by comma)



Send as email to: (text field, separate by comma)



Submit -> stored in DB, accessible through tab on user?s profile page?

e.g.

Class/Standards Earned

(Drop down menu) - (stars)

(Drop down menu) - (stars)

No I have not seen anything even close to that, so it is something you would have to have fully coded.
