Editor missing on default theme, buttons missing from custom theme?!

This forum is on 4.2.3 Beta 1.

I seem to have an odd situation where the enhanced editor appears to not be displaying on the vBulletin default theme. I have double checked my settings and in the admincp I have checked the settings under options "Style & Language Settings" and "Message Posting Interface Options" both are set correctly.

The interesting part is that the enhanced editor shows on our custom theme that we run all the time, however it is missing button icons but only for a handful of bbcode buttons, the buttons are still there just iconless?! (see screenshot below).

Anyone have any suggestions to fix the above issues?

Use ACP "maintenance" clear the vbulletin cache and recompile your styles. I think that will do it :)

This appears to be to do with the vaultwiki mod that I recently uninstalled, it seems to of not removed the buttons from the enhanced editor, so I need to find out how I can do this manually?

You would have to ask that in the mods thread where you got the mod from. :)
