Honestly don't even know how to explain this
Upgraded to vB 4.2 from 4.1 and at the top of EVERY page it this:

Then your problem is with your custom style. You said you have reverted all templates, but you haven't. Take a look at all the templates that are shown in red in your custom style.

If its at the top of every page then the header templates would be a good place to start looking.
