How to make Forums show Like Sub-Forums

I'm wanting to do something similar to the way that Child forums show under Forums with this Sub-Forum Mod, but for Forums under Categories.

Can anyone help me figure out what I need to do to make my forums look like this?

An example of what I'm wanting can be seen here.
