How can I use Image and Q&A verification at the same time?

We are having problems with spam bots and currently just use reCaptcha. How can I use Image and Q&A verification at the same time?

There is no setting to do that, and I don't believe I've ever seen any mod that lets you do that. But I think a lot of people are doing pretty well using just Q&A.
