Automatic Ban Lift Issue

Hi there. I've been having the following issue; whenever bans expire users get automatically moved back to their original usergroups, however when we look into their history it appears as if they were changed by random IPs leading to random users. Is there a way to fix this is just a common occurrence? Thanks a lot.
