Missing Avatar thumbnails on CMS widgets?

Ever since I upgraded to 4.0.8 my "mini" thumbnails are missing from widgets and blocks. (See screenshot). Even with all plugins disabled, defualt style and avatar rebuild, they are still missing.

Anyone have any clue how to fix or has anyone else had this problem? I have done searches on here and vb.com but I don't see anyone else with this problem.

In the screen shot is shows the default image, but I usually don't even get that. On firefox it is the username of the avatar, on IE8 it is a box with red x.
