Modify a current mod

Would anyone be able to change the above mod (Ted has given email permission)
What we are wanting is the mod to be used for members reviews on the forum.
Each member can then enter the username and add a report based on our gaming sporting history on the site and add info on there sportsmanship etc

As we use the mod in its standard form we would need it using different tables etc to the original mod

All the basic setup the same as it is now would be great but all the links pointing to said members profile etc once a review has been submitted
