Questions about attachment issue?

While trying to use attachments I've noticed since my upgrade to 4.1.5 that the thumbnails just stay as question marks. Is there a setting I need to change? Also when I insert inline it just gives me a link..not the actual image?

Also what is the default attachment folder in vb directory?
