Add a javascript table filter

Greetings, I'm using this hack

https://vborg.vbsupport.ru/showthrea...highlight=form

and it exports a results table. I'd like to add a row on top of the filter that allows the form to be filtered like this:

Can anybody help?

I can provide the code for the output table if necessary.
