Ticket System for Sticker Requests

I own an automobile forum and sticker requests can easily get out of hand. I would like to have a page where users can submit a "ticket" with their information and I can mark them "solved" once I send the sticker.

Are there any mod for this?

Usually this would be done with thread prefixes.
