Extending the registration process
 

In relation to the registration process, is the following possible?

This process should involve more than simply ticking a box to confirm acceptance of the privacy policy and terms and conditions. Members should be asked to sign up to some sort of application form in which they consent to their sensitive personal data (which in this case is likely to be information about their health) being used for the purposes of the Forum.

My thoughts were to add additional fields and tick boxes to the registration process but I would appreciate feedback and ideas?
