Show subscribed groups on homepage / widget?

Has anyone seen this or something similar?

I need a widget to display Social Groups that the user is subscribed to.

Just a simple list with Group Avatar and Group Name with link to the group.

I've been looking for a long time and haven't found anything for the homepage or widgets.
