Custom Thread? Is this possible?
 

I'm sure there is already a way to do this, but I just can't seem to figure it out to save my life. What I'm wanting is fairly simple. I'm looking for something so that when users create a "new thread" or "new report", they get a form with several options to pick from.

For example, click new report, towards the top, it'd have radio buttons with options to pick from. Kind of like how the prefixes work, except several of them assigned to this new thread.

Questions like:
What subdivision do you live in? Choices A-D
What was the crime? Choices A-D

Then the area for them to enter whatever they want. Then include the choices plus their own post information into the thread.
