Approved forum user, but posts are hitting moderation

I have a user that has been setup for about a year now. They are listed as "Approved Forum User" which has no moderation setup. For the last couple of months, most (not all though) of his post have to be validated by a moderator. Nothing has changed in the user profile. Any idea what may be causing this? What should I check for?
