Sidebar Login block

Thanks in advance for any help.

I'm trying to figure out how to add a forum block to the sidebar for users to login. If already logged in, it could give information about the user or just vanish, but if the user is not logged in I want it to look something like the attachment.

If I get this part working then I'll tackle one of the other things we would like in the sidebar.
