bread crumbs on custom pages

I know how to add the breadcrumb link in via my custom php page. My issue is that my pages are db driven. IE the page you are on is example " page.php?id=788". Is there a way to pass my id variable in the breadcrumb link so that it points back to the correct page Since you can't add PHP code directly.
