I need a very simple mod. It probably already exists but ive gone through 40 pages without any luck yet.

I need a simple form on the site that would post its information to a predetermined forum. Much like the guild application addons. However, the only data I would need in this form would be the users name, a date chosen by them and an option to select absent or late.

Simply used to allow guild members to let us know when they will be absent or late in advance so we can plan around them. Thanks!
