Problem with quickreply, thread and .....

After upgrading to 4.0.1 ->>> I tried to revert all templates, but nothing changed.

Probably some css. It's hard to tell from images. Did you revert your css templates?

Try this:
Create a new style with no parent:

Styles & Templates > Style Manager > Add New Style
Parent Style: No Parent Style
Title: Default vBulletin
Allow User Selection: Yes
Save

Then browse the site using that totally default vbulletin style - do you still have the same problem?

If it is still happening - Try disabling your modifications/plugins and see if you still have this problem.
Note: To temporarily disable the plugin system, edit config.php and add this line right under <?php

I tried everything, I reverted ALL TEMPLATES and used DEFAULT STYLE, but the problem remains unsolve, can someone help me?

But you didn't turn off plugins, that was also an important step, hence why it was suggested.
