User Profile Fields, Thread Prefix Problem

Checking to see if anyone else has had this problem before I submit it as a ticket...

When I create a New Thread Prefix or New User Profile Category and Field, it only partially shows up.

At first I thought it was just a matter of time for it to make its way through the process, but it has been several days now and the new prefixes and threads are not working.

Has anyone else encountered this problem? Is it a known issue and is there a fix for it?

I have attached screen shots from my Admin CP demonstrating the issue.
