Register form with date of birth window
 

How can I display this window while user is trying to register?
What exactly do I need to do? I already had had it, but then I changed some things and has dissapeared. I`m still learning .

This frame (registration window) shows up on the 1st step when the user presses Registration link (when he wants to register). And it`s before the forum Rules.
