To fight spam, I see many forum put a question like 2+2=? . What is this mod?

That is usually the question and answer human verification or a custom profile field that was added

It's not advised to use mathematical questions. Use more logical ones such as:
Q: What relation is my father's brother to me?
A: Uncle

Question and Answer, or reCAPTCHA?

I just found I can only choose one, not both.

Use reCAPTCHA as your Captcha then install a modification for Q&A.
Q&A is easier for users to answer as Captcha's can sometimes be difficult to read.
If your questions are good enough captcha isn't necessary.

Provided that you use questions that a bot could not easily answer, I'd think either should work fine. I had problems with bots and added about 5 random questions (you don't have to answer them all at registration, of course, it only asks 1 of the 5 available at random). I haven't had a single bot since.

But, I hear that reCaptcha is really good too. As far as I'm aware, bots can't read/answer it yet. Though, I don't keep up on that stuff, so you might want to research that for yourself.

If you want to use them both, you might be able to use a plug-in to provide one of them, then use the built-in vBulletin verification for the other (provided that you're using a vB version that has them). Perhaps Cyb's plug would work if it doesn't just use the built in question/answer system? Or, there is another one that people use....NoSpam, perhaps?

I was using reCAPTCHA but I received lot of complains from people that they can't put right word.
Then I swich to just simple picture and here is results.
With reCAPTCHA I had 60-70 new users per day with 0 spam
With simple picture I have 160-170 new users per day with about 1 spam message per day

I think I can handle with little spam for 100 new user each day :)
