How do I do this?

I am trying to figure out how to use the feature displayed in my attachment? Is this a default feature or did this site modify somehow?

I want to attach an icon with the name of the forum and the last post as they did.

Thats a hack, if i recall, to a search for "icons" in the modification forum, titles only. If i recall its a hack from the 3.6.x series, but works fine on any version.
