Double posts for no reason

Some of my users are double posting without meaning to, what could be the cause?

EDIT - Its happening randomly, not every time

Probably dial-up or slow connections...

Definitely not. Happened to me once or twice. Happening some of my users randomly, but its still annoying.

This is a real problem now. Its happening me a lot and another of my users.
